Role Overview The Nurse Practitioner or Physician Assistant will deliver home-based primary care to patients in their homes or Assisted Living communities. The provider will manage acute and chronic conditions, emphasize preventive care, and support improved long-term health outcomes. Key Responsibilities Conduct comprehensive in-home assessments Diagnose, treat, and manage acute and chronic medical conditions Perform physical exams, medication reviews, wound care, lab draws, and screenings Educate patients and caregivers on care plans and medications Collaborate with physicians and interdisciplinary teams Travel locally to patient homes during the workweek Qualifications Nurse Practitioner or Physician Assistant Minimum of 2 years of experience in Primary Care or Home-Based Care Active, unrestricted Indiana license NP certification (ANCC or AANP) or PA certification Experience in Family Medicine, Internal Medicine, or Geriatrics preferred DEA or ability to obtain post-hire BLS or ACLS certification Compensation & Benefits Competitive base salary Performance-based incentive compensation Medical, dental, and vision coverage with employer contribution Employer-paid life, short-term disability, and long-term disability insurance Generous paid time off, holidays, and wellness time Continuing Medical Education allowance Mileage reimbursement 401(k) plan with 4 percent employer match Apply Today Join a growing organization where patients come first, providers are valued, and teamwork truly matters .
