This facility is seeking a Primary Care Home Health Physician or Nurse Practitioner (NP) for locum tenens support as they look to fill a current need.

• Schedule: Flexible scheduling based on availability. (Part-Time or Full-Time basis)
• Job Setting: Home Care
• Types of Cases: In-home health and wellness evaluations
• Credentialing Timeframe: 4-8 Weeks
• Immediate start upon credentialing
• Must have an active Indiana state license
