The right candidate for this role will be dedicated, compassionate and a team player. GENERAL PURPOSEPerform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents ESSENTIAL JOB DUTIESObserve and practice safety according to facility policy and procedureAble to explain and demonstrate fire/emergency proceduresProperly use and maintain facility equipment.Demonstrate and able to explain infection control and isolation proceduresDemonstrate and able to explain proper body mechanicsEnsure residents are given correct dietUse proper feeding techniques while respecting personal dignity and meeting the residents' needs for socializationMake beds correctly and wrinkle freeMaintain residents' rooms in neat, orderly and clutter-free mannerSupport residents' participation in activity programsPromote and maintain positive relationships with co-workers, residents, visitors, volunteers, and regulatorsUtilize Care Tracker system for tracking daily resident caresDirectly respond, within their scope of practice, to needs and concerns of residents and family membersWork with the care giving team to identify and resolve resident and family issuesPromptly respond to call lights and other resident needsTreat residents with compassion and dignityEstablish effective communication/interaction with residents and familiesEnsure residents' right to privacy (visual and auditory)Maintain confidentiality of resident and facility records/informationShow respect for residents' personal propertyEnsure residents are dressed appropriately, consistent with weather conditions, and according to their wishesRecognize a resident's ability to make choices and support these within the constraints of the care giving environmentCommunicate with members of the care giving team about residents' personal choicesCommunicate residents' end of life concerns and wishes to the Charge NurseIs sensitive and appropriately responsive to each resident's end of life wishesPerform the following according to policy and procedure:Take and record temperature, pulse, respiration and blood pressureAmbulate residentsApply proper lifting and transferring techniquesProvide proper positioning and use of postural supportsMaintain residents' positioning, as neededWeigh residents on schedule and record weights accuratelyProvide restorative and rehabilitative nursingProvide active and passive range of motionImplement bowel and bladder programCollect certain specimens for cultures pursuant to facility policy and within the scope of dutiesEnsure residents' comfort while assisting them in achieving their highest practicable level of functioning as follows:Assist residents with their activities of daily livingProvide oral care and hygiene pursuant to plan of careProvide nail care pursuant to plan of careAssist residents with dressing and grooming according to their needs, wishes and desiresBathe residents with attention to privacy, respecting dignity and providing a safe and relaxing experiencePerforms work safely by consistently using gait belts, back supports and liftsReport the following to the Charge Nurse and the care giving team as appropriate and/or document according to policy and procedure: a significant change in a resident's condition, a change in a resident's vital signs or weight, any accidents and injuries, any changes in a resident's skin integrity, behavioral changes, signs of resident depression or anxiety, changes in a resident's ambulation status, the level of the resident's food consumption, and the resident's intake of food and liquids as well as the resident's output, when indicatedParticipate in collecting data needed for the accurate completion of the MDS and the plan of careShow understanding of information that is provided by the ADL flow sheetsProvide input to the Nurses for development of the plan of careProvide information gathered from care giving activities to the Charge NurseAssist in the orientation and training of other staffAttend orientation, mandatory in-service and education programs and staff meetingsComply with all Company policies related to safety and infection control procedures; including the proper use of mechanical lifts, gait belts and personal protective back supportsUnderstand, comply with and promote all rules and regulations regarding residents' rights; promote positive relationships with residents, visitors, and regulators, to include presenting a professional appearanceAttend and participate in educational activities, in-service training and staff meetings; assist in orientation and training other staffParticipate in Quality Improvement activities as assignedOther duties as assigned or needed CODE OF CONDUCTMust adhere to the Company's Code of Conduct and Business Ethics policy including documentation and reporting responsibilities QUALIFICATIONSMust be a Certified Nursing Aide (CNA) in good standing with the state. Must maintain a current certification in the state throughout employmentMust achieve 12 hours of in service education annually as required by federal regulations (as well as meet state requirements, if exceeds federal)Must have ability to read, write and follow oral and written directions at a level necessary to accomplish the jobMust be capable of maintaining regular attendance KNOWLEDGE, SKILLS, ABILITIES, AND BEHAVIORSDemonstrated ability to use computer and/or CareTracker systemMust be able to maintain confidentiality regarding patient and company proprietary informationMust be able to relate positively and favorably to residents and families and to work cooperatively and professionally with other employees at all levels PHYSICAL AND SENSORY REQUIREMENTSMobility, reaching, bending, lifting, grasping, fingering, fine hand coordination, ability to talk and hear; ability to read and write; ability to communicate with personnel, ability to remain calm under stress.
