The Hospice Clinical Manager is a registered nurse who manages the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. The Hospice Clinical Manager is responsible for the coordination of care during the hospice benefit period. The Hospice Clinical Manager is responsible for overall branch meetings, clinical practice, staff engagement, safety and the wellbeing of staff and patient care. My commitment will include:* Communicating significant findings, problems, and changes in condition or environment to the physician and other personnel involved with patient care.* Providing assistance for the development, implementation, and evaluation of policies, procedures, clinical protocols, forms, quality improvement monitors, service programs, and other components of program infrastructure every year and as needed.* Directing all aspects of operations and care delivery in accordance with the mission and values of Great Lakes Caring legal and regulatory requirements, payer and accreditation standards, customer expectations and agency policy.* Conducting performance appraisals accurately and as per policy.* Ensuring all documentation is accurate, complete and timely and adheres to Medicare guidelines.* Planning and implementing monitoring and reporting on the Hospice's services, specific clinical and administrative projects, and coordinating them with overall company objectives.* Coordinating services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 24 hours of referral.* Participating in the hiring and orientation of personnel. Qualifications:* Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing, BSN preferred.* Licensure: Current State License as a Registered Nurse preferred.* Experience: 2-3 years' experience as a registered nurse in an appropriate clinical care setting or home health environment preferred.* Skills: Nursing skills as defined by accepted nursing standards, preferred.
